Provides funding to cover the costs of prescription drugs listed in the Ontario Drug Benefit Formulary/Exceptional Access Program to Ontario residents who have high prescription drugs costs in relation to their income

Application form * call, visit website or ask for the form at pharmacy * must re-apply by September 30 of every year

Eligibility / Target Population:
|
Residents of Ontario with high medication costs, a valid Ontario Health Card and either do not have private insurance plan or private insurance plan does not cover all the costs of household's prescription drugs
