Service Description:

Medical health care team focused on family and community health *
coordinates patient care in order to assess and treat injuries or
illness * team includes family doctors, nurses, nurse practitioners,
social workers, pharmacists, dietitians, respiratory therapists, and
occupational therapists
Services provided only to patients of the doctor groups listed below
- patient access during extended evening and weekend hours for urgent problems
- after-hours telephone assistance
- assistance in navigating the health care system
- chronic disease management
- health promotion and education
