Service Description:
|
Community health care agency * Personal Support Managers available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home.
In-Home Personal Support Workers provide:
- Bathing/assistance with personal care
- Dressing assistance
- Mobility assistance
- Meal preparation assistance
Nursing services are provided in designated clinic locations. Please call our office for further information. Clinic nursing services include:
- Wound care
- IV Therapy
- catheter
- Drain/staple/suture care
- Health teaching regarding diagnosis/disease self-management/medication management
Nursing visits in-home are provided for patients who meet the clinic exclusion criteria.
