Four Counties Health Services
Four Counties Health Services (FCHS) is a primary care facility people providing emergency care, diagnostic services, and rehabilitation. Serving approximately 23,000 residents, it is located in the village of Newbury at the intersection of Lambton, Kent, Middlesex and Elgin Counties.
FCHS offers extensive outpatient care by provided by medical staff and visiting specialists, 24-hour access to a physician on-call for emergencies, continuing care and minor surgical services, an active and successful Adult Day Centre and supports numerous community programs such as Meals on Wheels, and VON’s Palliative Care Volunteer Program.
With 19 visiting specialists, FCHS provides emergency services close to home, following and helping to navigate patients through the spectrum of health care options.
Position: Registered Dietitian, Diabetes Education Program
Region: London and Middlesex
Description: The Middlesex Hospital Alliance is seeking a temporary, part-time Registered Dietitian to practice responsibilities within the Diabetes Education Program/Clinical Nutrition Department for up to 12 months.
This individual will provide Registered Dietitian support to Strathroy
Middlesex General Hospital and outpatients in our off-site clinics
within partner communities. Inpatient coverage may be required on an as
needed basis.
The successful candidate may work up to 75 hours bi-weekly. This position is available immediately.
Responsibilities:
- Within the outpatient environment- provides individual and group
nutrition assessment, counseling and evaluation of patients with
diabetes and pre-diabetes
- Within the inpatient environment- provides coverage on inpatient
units, completes nutrition assessments and develops nutrition care plans
for patients
- Completes written and electronic documentation
- Provides nutrition education to people with diabetes at various community outreach clinics and events/classes
- Special projects as assigned
Qualifications:
- Membership in the College of Dietitians of Ontario – required
- Valid driver’s license and unlimited access to vehicle for travelling to different clinic locations – required
- Demonstrates effective interviewing, written and verbal communication skills, including public speaking
- Demonstrates value and personal success in collaborative teamwork
- Demonstrates comfort and confidence in working independently
- Proficient in Microsoft Office, e-mail, and literature searches
- Certified Diabetes Educator designation – preferred
- Applied knowledge of enteral and parenteral nutrition therapies – preferred
- Applied knowledge of in Cerner power chart EPR – preferred
If successful, you must provide vaccination records or proof of immunity against measles,
mumps rubella and varicella (chicken pox) and provide proof of 2
COVID-19 vaccinations. You will also be required to provide documentation of the Tuberculosis skin testing within the previous 12 months.
Salary: $42.930 - $54.878 per hour
Terms of Employment: Temporary, part-time
