Hospital-based outpatient program for people with neurological, orthopedic, and cardio-respiratory conditions * healthcare team includes a physiatrist, physiotherapist, occupational therapist, recreational therapist, social worker, speech language pathologist, dietitian, and physiotherapy/occupational therapy assistant
Average length of program is 6-8 weeks
