To ensure this, the ministry works to :
- deliver better coordinated care in the community and closer to home by improving quality of life in long-term care homes
- ensure Ontario’s healthcare system is able to seamlessly transition older adults through various levels of care
- work with the Ministry of Health and partners to reduce long-term care waitlists across the province

Eligibility / Target Population:

All applicants must be 18 years of age or older * have a valid OHIP card * have specific care needs * have care needs which cannot be safely met in the community through publicly-funded community-based services and other care-giving support * have care needs which can be met in a long-term care home
