Service Description:

Teams provide comprehensive integrated services to adults living with a severe and persistent mental illness
- assessment
- individualized treatment plans
- coordination of care and services for clients
- one-on-one counselling and support
- life skills training
- monitoring of the mental state of clients and their use of medications
- vocational and recreational activities and programs
- family education and support
- referral and advocacy services to community agencies
