In-home personal supports for older adults, and adults with disabilities or chronic illness * helps people remain independent in their own homes for as long as possible * hourly, overnight, or 24-hour care
Custom care plans, such as in-home hospice, palliative and end-of-life care, respite care, hospital-discharge and post-operational care, or care associated with chronic conditions
- Alzheimer's and dementia care
- case management
- diabetic foot care
- eating assistance, total feeding
- friendly visiting, companionship, home help and homemaking, and errands
- medication administration or reminders
- meal preparation, menu planning, budgeting, shopping
- mobility, transfers, positioning (physical assistance with changing positions, moving around, getting dressed, and home exercise programs)
- nursing and therapy (wound management, ostomy care, tube feedings, physical therapy and rehabilitation, occupational therapy)
- palliative pain and symptom consultation programs
- personal care and hygiene (grooming, oral care, toileting)
- senior wellness coaching
- accessible transportation and escorts to medical and dental appointment
