Service Description:

Provides assessments, counselling, discharge planning services, information and education
Patients can counsel with social workers about:- adjustment to illness and treatment
- relationship and family issues
- stress and conflict
- grief and loss
- end of life care issues
- parenting issues
- discharge planning, making connections to hospital and community-based resources, and home supports
- communication needs and issues with the health care team
- issues related to physical, emotional and sexual abuse, and neglect
