Eligibility / Target Population:
|
People who require physiotherapy services, aged 65 years or older, or aged 19 years or younger, recently discharged as an inpatient of a hospital and in need of physiotherapy services that are directly connected to the condition, illness or injury for which they were admitted * eligible for funding of services under ODSP or Ontario Works
