Service Description:

Assistance for adults returning home from a hospital stay
Up to 3 hours of care on day of discharge * includes transportation from the hospital, pick up of groceries, medical equipment, medication, home management such as meal preparation, light housekeeping, laundry and follow-up call

People over 55, or younger people with complex needs who need assistance when being discharged from hospital
