Comprehensive integrated recovery-focused services to people with a serious mental illness
- assessment
- individualized counselling and support
- coordination of care and services for clients
- life skills training
- monitoring of the mental state of clients and their use of medications
- vocational and recreational activities and programs
- family education and support
- referral and advocacy services to community agencies

Eligibility / Target Population:
|
People with a severe mental health issue (ex, Schizophrenia, depression, or bipolar disorder) that has been diagnosed by a psychiatrist and has been disabling for some time and are aged 16 years and older and live in the Owen Sound area
