One-on-one appointments or workshops focusing on self-management skills, self-care, and topics related to diabetes
Team includes: - Registered Dietitians
- Registered Diabetes Nurse Educators
- Social Worker once a month

Eligibility / Target Population:

Adults 18 years and older with pre-diabetes, type 1 diabetes, type 2 diabetes and persons at risk for diabetes
