Service Description:

Mental health support to adults with a serious mental illness
Assists people to maintain a good quality of life while living independently in their community * works with people to maintain their residence and navigate social services such as Ontario Works and Ontario Disability Support Program
Services offered:- System navigation
- Supportive listening
- 1:1 and group appointments
- Assistance and support with basic needs
- Intensive case management
- Discharge planning
- Recovery and safety plans
- Psychosocial rehabilitation
- Education
- Consultation and collaboration
